LAS VEGAS (KTNV) — A chilling 911 call summoned Las Vegas Metropolitan Police Department officers to a Spring Valley home where, early Monday morning, one of them would be shot and two people would end up dead.
Asst. Sheriff Fred Haas revealing new details about the ongoing investigation during a press conference on Wednesday that included graphic 911 calls and body-worn camera footage.
Footage from inside the home showed a small glimpse of the harrowing experience that prompted a Las Vegas woman's desperate call to 911 just before 12:30 a.m.

When officers arrived at the home on Pigeon Point Court, blaring smoke detectors alerted them to a series of small fires inside. Haas says the fires were set by 48-year-old Jason Simmons, whose girlfriend called police to the home with reports that Simmons was "armed with a blowtorch and attempting to light her on fire."
Before police got to the house, the woman advised dispatchers that she had been shot and was hiding in a bathroom. She also warned that her teenage daughter was inside the house in one of the bedrooms.
Body camera footage played during the briefing shows officers arriving at the home and immediately trying to locate Simmons. They first find a girl inside one of the bedrooms who had been injured by gunfire.
Officers soon encounter a locked door an begin trying to kick it down. That's when gunfire erupts from behind the door, striking Officer Zaira Venegas-Hernandez in the chest. Venegas-Hernandez returned fire as police quickly backed away from the door.
At that point, Haas said, the situation was declared a barricade, and SWAT and crisis negotiators were called in. When they entered the locked room, Haas said they found Simmons dead from an apparent self-inflicted gunshot wound.
The woman who called police was taken to a local hospital to be treated for burn injuries and a gunshot wound. Haas said she remains in critical condition, and the teenager who was killed was her daughter.

One of the rounds fired at Venegas-Hernandez hit her ballistic vest and "went through 24 layers of kevlar," Haas said. She was treated at University Medical Center for a bleeding abrasion to her chest.
Venegas-Hernandez is 36 years old and has been with LVMPD since 2021, LVMPD officials said previously. She is on paid administrative leave while the shooting is under review.
Haas acknowledged that domestic violence-related calls are among the most dangerous situations for police and civilians.
"These officers ran toward danger. No matter how you look at that, they ran into a burning building to find somebody that was firing rounds at their family member and trying to light them on fire. It's not overlooked on us by what they actually responded to," Haas said.
This was the 10th officer-involved shooting for LVMPD in 2026, and came less than a week after the death of Officer Austin Abdelnabi.
Abdelnabi was killed in the line of duty on Aug. 4 while responding to calls about a man with a gun in the area of Tropicana Avenue and Mountain Vista Street.

They said they were told to contact Markthepharack whenever they wanted free tattoos.

A Las Vegas Metropolitan Police Department officer was arrested and booked into the Clark County Detention Center Wednesday on multiple charges. According to LVMPD, Antonio Chavez, 28, is charged with theft ($25K - $100K), four counts of burglary of a business and perjury.
