Nevada officials say three members of a state dental board have resigned, and two top staffers were terminated amid an investigation revealing multiple operational, systemic and ethical issues.

The Executive Branch Audit Committee announced Thursday that the 11-member Board of Dental Examiners is down to four members after being heavily criticized in an audit released earlier this year.

Republican Gov. Brian Sandoval ordered the audit before he left office after receiving multiple patient complaints about the board's response to their problems with dental care.

Executive director Debra Shaffer-Kugel and general counsel Melanie Bernstein Chapman were terminated and will leave Dec. 5.

Gregory Pisani, Jason Champagne and Byron Blasco resigned Thursday.

Democratic Gov. Steve Sisolak says he doesn't know if he would allow the remaining four board members to stay.
