Details released in Nov. 2014 prison shooting

More information has come out in a deadly prison shooting in November 2014 at High Desert State Prison.

Two inmates were in a fight when a corrections officer fired multiple rounds to break up the scuffle. Inmate Carlos Perez died while Andrew Arevalo was seriously injured. Both men were in handcuffs when shots were fired.

An internal Department of Corrections report finds corrections officer Jeff Castro is at fault as he failed to follow safety rules for escorting inmates and didn't break up the fight.

Castro is also cited for bringing "negative media attention" to the Department of Corrections.

Officer Isiah Smith was also found at fault because the report says he failed to separate the inmates.

Both officers resigned last year.

At this time, there is no report about the corrections officer trainee who fired the gun.
