LAS VEGAS (KTNV) — Clark County Department of Family Services has opened a case for investigation and a family assessment after a 2-month-old baby died in Las Vegas on Friday.

The Clark County Coroner's Office says Erin Lee Vandusen died on July 26 just before 11:20 a.m. He was 2 months and 11 days old.

CCDFS documents show the agency responded to 8 incidents with Vandusen's family since 2008.

Most of the incidents involved allegations of neglect or abuse and were found to be unsubstantiated, the documents show.

In September 2007, the family was placed under formal supervision after the department received allegations of abuse and neglect that is says were found substantiated.

The incident references multiple children, though it is unclear how many children the family has, saying the children were removed from an adult caregiver who they had been placed with as a result from the report.

The case was closed after the children were released into the care of the "appropriate parent," documents show.
