An elderly woman is dead after a crash on the freeway.

Preliminary investigation revealed that on Sept. 9, around 11:16 p.m., a gold 2004 Hyundai Sonata was on the 215 /Stephanie westbound on-ramp in the left travel lane.

For unknown reasons, the vehicle drove across the left rock embankment and entered 215.

The vehicle continued to travel left across all westbound 215 travel lanes causing the vehicles front to strike the center median barrier wall.

The sole female occupant of the vehicle was transported to Saint Rose Dominican Sienna Hospital.

On Sept. 10, the driver was transferred to UMC Trauma.

On Sept. 14, the driver succumbed to injuries sustained in the crash and was pronounced deceased.

The driver was identified as 90-year-old Phyllis Rossiter from Henderson, Nevada.

Investigators believe a medical episode may have contributed to the crash.

This marks the Nevada Highway Patrol-Southern Command’s 46th fatal crash resulting in 50 fatalities for 2016.
